A Dual Special Needs plan (D-SNP) is a type of Medicare Advantage plan designed for individuals who have both Medicare and Medicaid. Dual Special Needs plans are for people who could use more help. That may be because of income, disabilities, age and/or health conditions. Care coordination is another important benefit you get with a Dual Special Needs plan.
Chronic Special Needs Plans (C-SNPs) are a type of Medicare Advantage plan designed to cater to the unique healthcare needs of individuals with chronic or severe medical conditions. Eligibility: C-SNPs are tailored for individuals with specific severe or disabling chronic conditions. Care Coordinator or Case manager are often assigned to each member to help manage their chronic condition. Some of the chronic conditions include:
Chronic alcohol and other drug dependence
Autoimmune disorders (limited to specific types)
Cancer, excluding pre-cancer conditions or in-situ status.
Chronic heart failure
Dementia
Diabetes mellitus
End-stage liver disease
End-stage renal disease (ESRD) requiring dialysis
Severe hematologic disorders
HIV/AIDS
Chronic lung disorders (Limited to)
Asthma
Chronic bronchitis
Emphysema
Pulmonary fibrosis
Pulmonary hypertension
Chronic and disabling mental health conditions
Neurologic disorders (Limited to certain types)
Stroke
Institutional Special Needs Plans (I-SNPs) are a specialized type of Medicare Advantage plan designed to cater to people with specific healthcare needs. I-SNP's restrict enrollment to Medicare Advantage eligible individuals who, for 90 days or longer, have had or are expected to need the level of services provided in a long-term care (LTC) skilled nursing facility (SNF), a LTC nursing facility (NF), a SNF/NF, an intermediate care facility for individuals with intellectual disabilities (ICF/IDD), or an inpatient psychiatric facility.
